North East Regional Registration Form

Based off a large request to be included in the regional we have decided to do a "first come, first serve" entry.  If you want to be included in the regional you will do what ever is necessary to make sure you reserve your roster spot by promptly returning the entry form.  If you have not emailed me previously to be put on the roster your entry form will be put on a waiting list.
